What is on the form: three blocks
The standard Uber vehicle inspection form is a single page with three parts, and each part has its own owner:
- The vehicle operator block — yours. On the standard form this is short: your full name and phone number. That is the part you can type in advance.
- The inspection points — the mechanic's. Nineteen safety items, each with a PASS and a FAIL checkbox. The form warns that any marking on the FAIL side automatically fails the inspection.
- The inspector block — also the mechanic's. Vehicle mileage, license plate state and number, VIN, make, model, year, number of doors, the registration-sticker check, plus the inspector's company, address, name, signature, and state certification number.
One caution before you type anything: the form is not identical everywhere. Some states use their own version or their own state inspection instead, and requirements change by city. Always start from the copy Uber provides for your market.
The 19 inspection points — check them yourself first
Here is what the mechanic will mark pass or fail on the standard form, grouped so you can walk around your car the night before:

The cheap failures are the common ones: a blown brake-light bulb, a worn wiper blade, a horn that stopped working. Each costs a few dollars to fix at home — and a second inspection visit if the mechanic finds it first. Ten minutes with this list can save you a repeat fee.
