What a direct deposit form does
A direct deposit form — often called a direct deposit authorization — tells an employer or payer where to send your money electronically, straight into your checking or savings account, instead of handing you a paper check.
There is no single official version. Employers, payroll providers, banks, and government agencies each hand out their own layout, so you complete whichever PDF you were given rather than a standard IRS-style form.
What the form asks for
- your name and, sometimes, your employee or ID number;
- your bank or credit union name;
- the account type — checking or savings;
- the bank's routing number (9 digits) and your account number;
- your signature authorizing the deposit.
Some forms also ask you to attach a voided check so the payer can confirm the account.
Where to find your routing and account numbers
On a paper check they run along the bottom edge: the 9-digit routing number comes first, then your account number. If you do not have checks, both numbers are shown in your bank's mobile app or online banking under account details.
